Welcome medical students!
 
Welcome to the Department of Anesthesiology, Perioperative and Pain Medicine at Texas Children’s Hospital! This rotation (selective or elective) has been designed with the needs of Baylor College of Medicine students, and visiting students, in mind.  During this rotation you will receive supervised instruction in the principles and techniques of anesthetic management of children undergoing surgery at Texas Children’s Hospital (TCH) Operating Rooms.  At TCH, over 30,000 anesthetics are performed annually, for surgical, therapeutic and diagnostic procedures. 
 
When you arrive on the first day of your rotation, please report to Carissa Johnson in the Anesthesiology Office at 8:00 AM.  She will help you with obtaining your ID and completing necessary paperwork.  Following this, please find Dr. Aina in the operating room, or call the Voalte phone (x30752), before you leave for the first day’s assignment.  Your first day will be spent in the PASS (Pediatric Anesthesia Screening Service) clinic, to get acquainted with the preoperative anesthetic evaluation. On subsequent days, your clinical assignments will be in the West Tower, or the Mark Wallace Tower.  You are expected to find your assigned attending for the day around 7:00 AM, either in the preoperative holding area or operating room.  Your daily schedule will be from 6:30 AM to approximately 4:00 PM.  There will be no night or weekend calls.

The reference textbook for this rotation is Miller’s - Basics of Anesthesia, 7th edition.  You may borrow this book from Carissa Johnson.  Please return all loaned books at the end of your rotation to Carissa, otherwise your evaluation will be delayed until the items have been returned.
 
Goals and Objectives
 
You will be given an opportunity to attain the following goals and objectives:

Knowledge:
  1. Understand the basics of the anesthesia machine, ventilators and routine monitors (pulse oximetry, capnography, blood pressure, and electrocardiography).
  2. Pharmacology of anesthetic drugs including muscle relaxants, sedatives, volatile anesthetics, induction agents, narcotic and non-narcotic pain medications, local anesthetics, antibiotics and other related drugs.
  3. Various pain management techniques used in intra operative and postoperative periods.
  4. Induction, maintenance and emergence from anesthetics.
  5. Fluid management, hemodynamics and respiratory effects of anesthetics and surgery.
 
Clinical skills:
  1. Ability to preoperatively evaluate a patient’s history, physical and laboratory data critically from the anesthesiologist’s perspective.
  2. Evaluate a patient’s airway. 
  3. Ability to perform peripheral venous cannulation, bag mask ventilation, laryngeal mask airway placement and endotracheal intubation in routine cases.
 
Communication skills:
  1. Communicate effectively and professionally, while developing a good rapport, with the family and the patient.
  2. Communicate effectively and professionally with the other operating room staff.
  3. Effectively lay out a plan and describe the anesthetic management of healthy patients.
  4. Discuss the cases during the operating room case management.

​Didactic Conferences
 
During this rotation you are expected to attend didactic conferences Monday, Wednesday, Thursday, and Friday at 6:30 AM in the individual attending offices (Abercrombie).  On Tuesdays, the Pediatric Anesthesia Fellow Conference will be held at 6:15 AM in Dunbar Library (A200.43), and the Quality Improvement (PAQIC) conference is on the 4th Friday of every month in Dunbar Library (A200.43) at 6:15 AM.
 
Expectations
 
  1. Attend didactic lecture series.
  2. Read all assigned reading materials prior to each lecture.
  3. Discuss with your assigned anesthesiologist any topics of interest and your assigned lecture for the day.
  4. Be present in the assigned operating room in a timely manner, and be ready to assist in preparation of the room.
  5. Be present for preoperative assessment and consent, introduce yourself to the family, accompany patient to the operating room.
  6. Be available to help with all aspects of anesthetic care of your cases, including drug preparation, line placement, positioning, induction, and intraoperative monitoring, emergence and transport to PACU.
  7. Be prepared to do procedures (IV placement, bag masking, Intubation), based on ability and experience.
  8. Understand the importance of preoperative assessment and its clinical relevance.
  9. Understand anesthetic management of healthy patients for simple procedures.
  10. Understand post-operative concerns and pain management.
 
 
Final grade
 
Students will be evaluated by faculty, fellows, and residents.  This evaluation will be based upon clinical performance, attendance, fund of knowledge, attitude and level of professional maturity.  A final exam will be given on the last day of your rotation.  You will also get an opportunity to evaluate the rotation and contribute with your suggestions.  For students on the 4-week rotation, an oral presentation of about 15-20 minutes in length will be required.
